New Patient Information

In order to book an appointment, you must have your COVID-19 Form and New Patient Intake Form completed. Once completed, please contact us or we will contact you Monday through Friday to book an appointment.

We are a private clinic, therefore a majority of our billing is done privately. We charge an office visit fee of $85. There are additional charges depending on the treatment plan. For virtual visits, we charge $95.
